One of the most common physical insecurities for men is penis size. Most men have penises that fall within an inch or so of average, but that is little consolation for those whose penises are on the shorter side of it. And it doesn't matter what age you are or how accomplished you feel in every other area of your life — if you don't measure up below the belt, it can devastate your self-esteem. Conversely, a larger-than-average penis appears to improve self-esteem in a measurable way.1
There's an entire industry based around this concern, even though most women in heterosexual relationships report being satisfied by their partner’s size. Still, most men believe that their penises are "smaller than average" — and a bevy of companies promise that they have the products you need to increase the size of your penis along with your self-esteem.3
Phallosan offers a product called Phallosan Forte, which it claims will significantly enlarge your penis in both the short and long term. But will it get the job done? Below, we break down whether Phallosan Forte is worth the investment.
From our testing and research, Phallosan Forte is effective. It can increase penis length, and while gains in girth are likely to be less pronounced, it may increase girth slightly more than extenders that rely on traction alone. It's based on a well-researched method typically used for correcting extreme cases of penile curvature that also tends to add length to patients’ penises. The device is among the more comfortable extenders to wear for long stretches, even in day-to-day life or while you sleep. It takes a little time to get used to the order of operations, but once you have it down, you'll be able to put it on and take it off with ease.
At Innerbody Research, we extensively test each health product we review, including Phallosan Forte. Our team has accumulated thousands of hours testing and researching penis extenders, comparing the top products on the market, and poring over available clinical research. To date, we’ve read more than a dozen clinical research studies examining the effects of penile traction. The field demands further research, and we’ll stay on top of developments as they occur.
In addition to research, our team got hands-on with these devices, experiencing everything from the purchasing process to what it feels like to wear one for an extended period. Additionally, like all health-related content on this website, this guide was thoroughly vetted by one or more members of our Medical Review Board for accuracy.
Over the past two decades, Innerbody Research has helped tens of millions of readers make more informed decisions to live healthier lifestyles.

Whenever we evaluate a penis extender, we look at a handful of specific criteria to determine its value as it compares to similar options. In this case, we concentrated on effectiveness, cost, convenience, privacy, and customer support.
By combining suction and traction, the Phallosan Forte offers one of the most effective devices on the market. Typical penis enlargers rely solely on traction, which, while effective, typically has minuscule impacts on girth.4 And girth is often cited by women as being more important than length for their pleasure.
Because it combines traction with vacuum pressure, the Phallosan Forte has the potential to enhance both length and girth. According to a recently published study in The Journal of Sexual Medicine, six months of using both traction and vacuum pressure can result in up to a .85-inch increase in length and a .47-inch increase in circumference.5 It’s worth noting that the study also employed a platelet-rich plasma therapy that may have influenced growth rates, but the source of the growth was the use of devices.
Penis enlargement is not a very comfortable process. Even with the Phallosan Forte, which is more comfortable than most, you have to wear the device for several hours daily over the course of several months to see results. As a result, we grade the convenience of any penis enlargement device on a bit of a curve.
The Phallosan Forte's comfort is its most impressive feature. The protective sheaths are easy to put on and allow your penis to enter the suction bell with little to no noticeable sensation. This design is a big improvement compared to basic traction devices that can constrict blood flow around the glans.
Using a belt to manage traction force also allows you to engage the device with a greater degree of ease and to make adjustments to its tension before it has the potential to cause you any discomfort. That belt also allows you to position the device in more directions than a typical traction device would allow. This makes it easier to hide under clothing and better for maximizing your potential comfort if you wear it while sleeping.
The PeniMaster Pro is the only other model we would recommend that has a similar form and function, employing both vacuum pressure and traction. But that system gets confusing very quickly compared to the more straightforward application and simple packaging you get with Phallosan Forte.
Phallosan Forte might have rated a little higher in value if its price point were more forgiving. Compared to products like the Quick Extender Pro, which is also effective, Phallosan’s prices are a significant increase.
Here’s a look at how Phallosan’s costs compare to other top extenders:
Cost | Traction or vacuum system | |
---|---|---|
Phallosan Forte | $379 | Both |
PeniMaster Pro Complete Set | $350 | Both |
Quick Extender Pro Value Edition | $120 | Traction |
Quick Extender Pro Deluxe Limited Edition | $350 | Traction |
SizeGenetics Value Edition | $199 | Traction |
As you can see, traction-based devices are typically less expensive, and the Phallosan Forte is more expensive than the PeniMaster Pro Complete Set, which utilizes traction and vacuum pressure.
You also get a lot of high-quality materials for the money compared to average traction packages, but they aren’t all necessary for every customer. The Phallosan Forte comes with bells in small, medium, and large sizes. When the device arrives, you measure your penis with an included tool to determine which bell you should start with. And if you don't need the small to start, you’ll never need it, meaning you're essentially paying for materials that you won't use.
There is no way to measure your penis before ordering, nor is there a way to customize the package and save money. If the company started selling its measuring device (or even offering it for free) and allowing customers to augment the contents of their shipment, its value would markedly increase.
It's tough to misuse the Phallosan Forte. It may be challenging to learn how to use it, but there aren't a lot of opportunities to hurt yourself along the way. We include a video in the section below to help you get the hang of it; once you do, you can rest assured that the science behind the technique is sound.
The most helpful safety feature can be found on the tension bar. As you tighten the belt that increases the tension on the device, the inner portion of the bars reveals itself. A color guide with green, yellow, and red sections is printed on that inner bar. That provides a quick and handy visual reference that you've dialed in the tension correctly (or incorrectly).

The real potential for danger with Phallosan Forte comes not from possible misuse but rather from a lack of care for the device and its materials. You’ll want to clean the Phallosan Forte regularly, especially the inside of the vacuum bell. This closed space can potentially breed bacteria, and even though your glans is protected by the included protector caps, any damage to those could increase your exposure.
The protector caps themselves also pose a potential hazard. They come with a layer of a slick talcum-like powder already applied. This powder layer wears off after 2-3 uses, and if you don’t reapply it, you can experience significant chafing on the glans.
Ultimately, these protector-cap and vacuum bell-related issues aren’t a problem with products like the Quick Extender Pro, which relies only on traction and has no enclosed components. But at the same time, the additional bells and whistles of Phallosan Forte are also what make it capable of maximum comfort and even night-time usage. You just have to invest proper time to care for the equipment.
Phallosan's customer service still needs some work. It offers fast shipping and a relatively intuitive website, but its written materials are poorly translated. This applies to the included instruction manual, which, at times, seems like it could be clearer.
Communication from the company could be better, as well. Phallosan Forte doesn't provide any shipping or tracking information after your order, so you'll have to keep an eye out for the delivery. Our experiences with Quick Extender Pro were much better, as the company employs a chat feature none of its major competitors can boast.
Phallosan Forte is a penis-enlarging device made by Phallosan, a German medical company specializing in men's health. It uses a combination of traction and suction to safely and effectively stretch your penis, which will promote growth and cell regeneration if used consistently over several months.5 You can also use it to treat Peyronie's disease (curvature of the penis) as well as size and potency loss resulting from prostate surgery.
It’s one of the most comfortable enlargement devices our testing team has encountered, and its belt system makes it more convenient than most others if you want to wear it while sleeping.
Check out our video below to get a look at the components that come in the Phallosan Forte package and how they fit together to the form the assembled device:
The short answer is "yes." When researching penis enlargement treatments and devices, it's essential to take any statement a company makes with a fistful of salt and a healthy amount of research. Search for "penis enlargement" online, and you'll find a cornucopia of shady companies with names like pornographic superheroes that promise you three inches of growth in two weeks through some combination of snake oil and pseudoscience.
Phallosan Forte, on the other hand, uses tested methods for penis enlargement. You aren’t going to gain three inches any time soon, but you can expect gains approaching an inch in length after about six months.
We do wish Phallosan offered a more robust money-back guarantee beyond its 14-day return policy. Quick Extender Pro has one of the best such guarantees, allowing you to use its device for up to six months and still return it. That said, Phallosan has one of the longest warranties of its kind, offering two years of protection against defects.
Phallosan has no reputation with the Better Business Bureau, likely due in part to its headquarters being located in Europe. The company does have a profile on Trustpilot, however, and reviews are mixed.
Phallosan’s Trustpilot rating is currently a 3.1 at the time of this writing, with 29 reviews in total. This isn’t enough to reach statistical significance, but it’s worth investigating the collection for recurring themes. The bulk of customer complaints arise from issues with customer service, which we touched on briefly above. The company can be slow to respond to emails, and contacting them by phone is complicated from the U.S. due to the time difference.
The principle behind Phallosan’s penis enhancement system is the same principle behind muscle-building in weight training.10 When parts of your body experience tiny microtears, your body's natural cell regeneration processes take over. When you apply this concept to your penis, you can increase length and girth. The Phallosan Forte can apply up to 3kg (6.6lb) of traction to your penis. This can create increases in length and girth after about six months of use. If you want to see more immediate gains, you'll need to wear it for at least 6-8 hours a day.
Phallosan Forte differs from the vast amount of penis enhancement devices in that it doesn't use a clamp to separate the head of your penis from the shaft, which can cut off circulation to your penis. Instead, the device uses a suction cup on your glans that's carefully attached to a strap. That strap fits around your waist, holding it in place for up to a maximum of 12 hours per day. Since it doesn't cut off circulation to your penis during use, many customers can comfortably leave the device on while sleeping.
Over time, painless, microscopic tears will occur in the shaft of your penis, which your body will repair using cell regeneration. With consistent use, the new tissue created in this process results in a longer, thicker penis.
Every piece of equipment you need comes included in the Phallosan Forte kit, and it also comes with an extensive instruction manual. Unfortunately, this manual was poorly written (and poorly translated) and is more likely to confuse users than help them. It took our testers an average of two full read-throughs before they were comfortable trying on the device, and even then, they took it slowly.
We’ve put together a video using a model of a flaccid penis to give you a clearer sense for how this device works and how to begin using it:
Here is a breakdown of the steps:
As with any piece of training equipment, gains in penis size will be consistent with the amount you've used it and the way you've used it. A lot of users have reported that if you use Phallosan Forte consistently for at least 6-8 hours a day, you could see consistent gains within the first couple of months — after over a year of use, users have reported gains of one to three inches of length, along with girth increases. While clinical research hasn’t borne such results, most studies conclude within about six months.5 Research into the long-term efficacy of traction and suction is lacking, so we have only anecdotal reports.
Insider Tip: You’ll want to get your hands on some baby powder for maintenance. When the powder that comes pre-applied to the protector cap and sleeve wears away, chafing can become a serious issue. A sprinkle of powder every few uses will alleviate this.
Traction devices have been investigated as potential remedies for Peyronie’s disease for some time.6 Clinical research has been successful, and much of this research forms the basis for claims that traction devices can increase penile length and girth, as well.
Peyronie’s is a significant curvature of the penis, so much so that it makes intercourse difficult or painful. Severe cases interfere with daily life. By stretching the penis out in a straight line, traction devices slowly reverse this curvature, resulting in a straighter penis that’s more practically functional.
The Phallosan Forte should work exceptionally well for Peyronie’s disease thanks to its directional flexibility.7 Typical traction extenders limit your ability to position the penis in one direction, but Phallosan’s belt system allows you to hold the penis at extension in various positions. That might allow you to position your penis in a direction counter to its curvature. Of course, this would have to be done under the supervision of your physician.
While urologists often prescribe the use of traction devices to treat Peyronie’s disease there are some other alternatives. Collagen-dismantling injections directly administered to the site have been shown to be effective, particularly in conjunction with traction.11 And an oral medication called Xiaflex can help, as well, though that company’s own list of side effects includes penile corporal rupture, which can result in erectile issues and significant pain.
Phallosan Forte is entirely safe to use if you take it slowly and follow the instructions as closely as possible. Of course, that would be easier if the instructions weren’t poorly translated from the original German, but we’ve included a short version of the instructions above, along with a handy video.
If you apply too much force with the Pallosan Forte or wear it for too long a period, you run the risk of causing some bruising and mild to moderate tissue damage. If you experience any pain while wearing the device, you should loosen the belt slightly to reduce the tension force. And if pain lingers after you’ve removed the device, you should take some time off until you’ve healed and are pain-free.
One risk this penis extender poses to men comes from its condom sleeves and protector caps. These come pre-coated with a substance similar to talcum powder, and you’ll want to reapply baby powder of your own after every 2-3 uses. As this powder coating wears away, chafing inside these rubber components can cause skin irritation or even tearing. You also want to keep all of the components clean and replace condom sleeves and protection caps if they tear, as those openings could harbor bacteria.
Compared to traction without the vacuum pressure, the Phallosan Forte isn’t more or less safe. There isn’t much additional risk from the suction component, and the basics of the tension systems are fundamentally similar.
More important is the fact that vacuum pressure and traction are both significantly safer than surgical alternatives. Penile enhancement surgery, whether for elongation or increased girth, is a dangerous prospect with a poor track record.8 In some instances, the base of the penis is severed from the pubic bone, which typically anchors it and provides you with greater control during intercourse. Men who sever this connection may seem somewhat longer, but the tradeoff in control is noticeable, and there are cases in which the penis actually retreats somewhat into the body, nullifying the intended effects.
Girth enhancement surgery is even scarier. Surgeons will often make an incision in the penis and peel back the skin, inserting a layer of tissue between the muscle of the penis and the skin, then sewing it back up. Cadaver tissue has commonly been used here, and rejection rates are higher than most men would allow. Necrosis of the skin and infection are real risks.9
Ultimately, Phallosan’s safety profile is head and shoulders above surgery, and we advise any man interested in the latter to try the former first.
Phallosan’s product lineup is decidedly simple compared to some others. There aren’t too many packages or accessories to choose from. Let’s take a quick look at its offerings:
The Phallosan Forte kit costs $379 and includes all of the equipment you're going to need to begin your penis-enlargement routine:
For around $20 less, you could get the PeniMaster Pro Complete Set, which works similarly to the Phallosan Forte. But the PeniMaster offers significantly less pulling force in its Complete Set — 1,150g compared to Phallosan’s 3,000g. To increase the pulling force of the PeniMaster, the company sells an attachable weight set, but we aren’t fans of this configuration. It’s too much to store, too complicated to set up, and it increases the risk of injury from misuse.
Phallosan offers a new additional option as well: the Phallosan Plus+. You can purchase Plus+ along with the Phallosan Forte — a combination package costing $499 — or by itself afterward for $120. You do not save any money purchasing them together.
You use the Plus+ along with your Phallosan Forte for additional traction and faster or extended results. Compared to the 3kg (6.6lb) of force the basic Forte applies, the Plus+ delivers 4.8kg (10.6lb).
Based on our research, 3,000g should be enough traction force for most men, but higher forces may be necessary in cases of Peyronie’s disease. This is something you should discuss with your doctor before trying at home. And since buying the Forte and the Plus+ together doesn’t net you any savings, we recommend most men wait until they’ve tried the Phallosan Forte before spending any extra money on the system.
Shipping from Phallosan is free in the U.S., and it typically takes 2-3 business days. You'll get order and processing confirmations but no tracking information or delivery confirmation from the company.
Once you receive the device, you can only return it if it's unopened. There's a thin cellophane wrapping around the product box, so there's little chance that anyone could try the device and then return it. Be careful not to tear this cellophane if you aren’t sure you plan to keep the device, as this is your only proof that it was unopened.
If there are any defects with the product, you'll have to submit photos along with a replacement request. Phallosan reviews these on a case-by-case basis and has no firm policy to reference. Based on customer reviews we’ve read, this process can be frustrating, and Phallosan isn’t always quick to validate claims.
That said, most penis extension companies have similarly spotty track records regarding warranty requests. Our testers had a similar experience with a company called SizeGenetics, which made us jump through several unnecessary hoops after our shipment arrived with missing parts. Our shipments from Phallosan have been intact and fully functional, so we’ve yet to test the company’s warranty process.
Phallosan Forte is committed to safeguarding your information.
Like most companies with an online presence, Phallosan uses cookies to track your browsing data around your use of its site. While this is a very common practice, it’s worth noting that Quick Extender Pro explicitly states that it does not use cookies, which we greatly appreciate, given the sensitive nature of these products.
Like the Phallosan Forte, the PeniMaster Pro combines suction and traction to provide a comfortable and effective extension. Its prices are competitive with Phallosan as well. The company has more packages available, so you don't have to start with the most expensive offering to test the waters. Conversely, those packages get slightly confusing and more expensive than the Phallosan Forte.
From a quality standpoint, the two are neck and neck, but Phallosan’s device is capable of significantly greater traction force than the PeniMaster Pro without its optional added weights. While the PeniMaster Pro can reach around 1,150g (2.5lb) of force, the Phallosan Forte reached 3,000g (6.6lb) while being easier to use.
Check out our PeniMaster Pro review for more information.
Quick Extender Pro is our favorite offering among simple traction devices. It has a high build quality, a surprising level of comfort for its class, and an informed and responsive customer service team. With the Quick Extender Pro, you can continue adding extension rods as you grow past your goals, and those additional accessories are rather affordable.
While they don’t boast vacuum pressure, all of Quick Extender Pro’s models are as strong or stronger than the Phallosan Forte when it comes to tension force. The QEP Value Edition is as strong as the Phallosan Forte, providing 3,000g (6.6lb) of force, while the Deluxe Standard Edition offers 3,500g (7.7lb) and the Deluxe Limited and Peyronie’s Editions offer 4,000g (8.8lb).
Learn more in our comprehensive review of Quick Extender Pro.
SizeGenetics bears striking similarities to Quick Extender Pro, but the companies behave extremely differently. The SizeGenetics customer service experience is protracted and agonizing, and if you’re lucky and persistent enough to have your complaints heard, you’ll find resistance to even the most basic solutions. This is all a shame because the product is well-made and would be a fantastic budget option for anyone looking to experiment with traction to see if it’s right for them. To learn more, visit our SizeGenetics review.
If you’re into luxury products, then Jes Extender might be the choice that will bring you the most joy. Its products often contain precious metals and other aesthetic upgrades that significantly increase costs. Below the shiny surface is an effective extension device that won’t disappoint you, but there is one catch — even for those to whom money is no object. The increments between extension rods are slightly larger than between rods from other companies. The result is more significant jumps in extension length with each increase, which can lead to discomfort or bruising in especially sensitive users.
Thanks to its combination of vacuum pressure and traction force, the Phallosan Forte should be able to add length and potentially girth to men’s penises. The effects take time, patience, and commitment, and gains won’t be significantly greater than you’d see from using a less expensive traction-only device. But the comfort, relative convenience, and growth potential the Phallosan Forte offers are among the best in its class.
