Studies tell us that 95% of men have erections whose length is less than 6.3 inches, and a full 85% of men have penises that fall roughly within an inch of average length when erect. In the broader world of penis enlargement, approaches commonly fall under three different categories — surgery, substances, and devices. Penis extenders fall under that third category of products. All of the penis extenders in this guide rely on a method of penis enlargement called traction, or penile traction therapy (PTT).

Extension devices were intended to treat Peyronie’s disease, a condition in which calcified tissue accumulates in the penis, causing painful curvature.3 These devices were effective at straightening the penises of men with Peyronie’s disease, and study subjects also saw average increases in length just under one inch.4

Traction as a method of enlargement

The easiest way to understand penis traction is by thinking of muscle-building. When people lift weights or work out to build muscle, the exercise causes very tiny tears in our muscle tissue. The body repairs these microtears with cell replication.5 This repair process is what builds our muscles.

In essence, this is the concept behind the effectiveness of traction in enlarging the penis. A traction device, such as a penis extender, applies pressure to stretch the tissues of the penis gradually but persistently for several hours at a time. This pressure causes microtears in the tissues of your penis, which the body then repairs similarly to how it repairs and "builds" muscle after a proper workout. Gradually, this translates to growth in penis size.

As the body repairs microtears in the corpora cavernosa — the spongy tissue in the shaft that collects blood — men find their erections gaining noticeable size and strength. Clinical studies confirm that wearing penis extenders produces actual results for men hoping to enlarge their penises.6

Importantly, the growth in size that some men see within the first couple months of using traction-based devices may not be permanent. But studies show that the gains become permanent after using a traction device for months rather than weeks. Once you have used a penis extender for several months, you can stop when satisfied with your size, knowing that the gains are now permanent.

How big can you get?

While it’s fairly common for companies to suggest gains of multiple inches within the span of months, we believe these claims are exaggerated. Most studies we've read have used traction devices for anywhere from 12 weeks to 12 months. But we have yet to encounter a credible study in which participants gained more than an inch or so in length. That said, anecdotal reports convey tales of greater gains, particularly from men using penis extenders for years.

We can confidently say you can expect to gain upwards of an inch in as little as 12 weeks, but more likely over the course of many months. Other factors may play into whether greater gains are possible in certain individuals, but no scientific studies have illustrated this possibility.

Penis extenders are a suitable choice for nearly any man who wants to add length to his penis, along with potentially a more modest gain in girth at the glans (or “head”). Just make sure that your expectations are realistic. All research, testing, and customer interviews that we’ve done lead us to conclude that your realistic expectations for success are in the 1-inch vicinity after months of using a penis extender. After using the device for several months, those gains are permanent and certainly dramatic enough to satisfy most men. If you’re one of them, then a penis extender could be a great option for you.

Using a penis extender for Peyronie’s disease

Men with Peyronie’s disease often find that traction-based penis extenders are among their most effective options. Urologists may combine traction with other therapies, including oral medications and penile injections, both of which carry some significant side effect risks. But traction is typically a cornerstone of treatment.

Treatment regimens may look different when using a penis extender for Peyronie’s than they would for enlargement alone. For example, sessions with the Quick Extender Pro add an extra centimeter of extension from the start, increasing the tension force on the penis. Men with Peyronie’s often experience straightening and extension concurrently. You should talk to your doctor about using a penis extender to treat your Peyronie’s before attempting treatment on your own.

What do paring knives and penis extenders have in common? Hopefully not much, but there is one thing: sometimes the least expensive choice can be the best for most people.

The market for penis extenders is crowded, and not all devices are the same. Based on our testing and analysis, Quick Extender Pro gives the best combination of strength, comfort, affordability, and fast results. For that reason, it’s our editor's choice as the best penis extender for most men in 2024. It's also one of the most popular products among the men we've interviewed in the process of making this guide.

First, let’s consider strength. The Quick Extender Pro is capable of greater tension than almost all of the high-quality penis extenders we tested. At 4,000g tension strength, only a very expensive combination of Phallosan Forte products would exceed it. This high amount of tension strength and the classic approach (sans vacuum) make this our recommendation for correcting mild to moderate Peyronie's curvature, as well. RestoreX, a model advertised specifically to combat Peyronie’s, costs over $500 and is significantly less comfortable than Quick Extender Pro.

In terms of comfort, you could spend more money for a more comfortable penis extender — namely, the Phallosan Forte — but we find the Quick Extender Pro to be plenty comfortable for its purpose. (This isn’t a device that is intended to be worn overnight, for instance; if that is a requirement for you, then consider the Phallosan Forte or PeniMaster Pro.) With a dual-strap approach and adequate padding, it’s also more comfortable than other competing rod-based penis extenders that cost more.

Though one of the strongest penis extenders on the market, the Quick Extender Pro is lightweight and small enough to be worn pretty discreetly under baggy clothing in very low-activity public settings as well, in a pinch — something we were able to verify in testing.

Quick Extender Pro offers several products at different price points. Right now, you can buy a high-quality, rod-based penis extender from this company for less than comparable products elsewhere, making it an excellent value.

Below is a quick breakdown of available products. For most people, we believe the Deluxe Standard Edition will be the best value.

Quick Extender Pro offers a 6-month satisfaction guarantee that asks you to use the penis extender for six months and then gives you 20 days after that to set up a return. All of the other traction-only devices in our guide offer 6-month guarantees, as well, but they require you to submit before and after photos and enter your measurements on a weekly basis on the parent company’s “Diary.” We consider this to be both invasive and an unfair loophole buried in the fine print that will nullify almost every customer’s guarantee. Quick Extender Pro has no such loopholes or invasive requirements.

However, Quick Extender Pro’s warranty isn’t the longest in the business. It lasts for a year, which is long enough for you to gain upwards of an inch, but companies like Phallosan offer 2-year warranties on their products.

Best for convenience, best for maximum comfort, and best for men with micropenis

The Phallosan Forte stands out in a few ways from many competitors. It relies on both traction and vacuum mechanisms. For some of you, its combination of features and convenience ought to make it a top consideration. It’s one of only a few penis extenders on the market you can wear overnight. It’s harder than other devices to detect under clothing, and its combination of vacuum pressure and traction allows it to increase both length and girth.11 Traction-only penis extenders typically do not offer much girth enhancement.

Our testers were impressed by the Phallosan Forte. In the video in the “How to use a penis extender” section below, you'll see that it's relatively easy to put together and wear, once you've figured out how each part works. Unfortunately, the instructions are worded poorly, likely due to their translation from the original German. (Phallosan's parent company is located in Germany.) But between those instructions and our video tutorial, you should be able to master it shortly.

The overnight treatment option is tantalizing for obvious reasons. Many penis extenders just don't do well in this scenario either because of a bulkier, cumbersome design or because they become painful when a nighttime erection happens. We found that the Phallosan Forte is one of two penis extenders that excel in overnight wear. It's capable of delivering this level of convenience thanks to the way the apparatus hugs the side of the waist, making things less frustrating for those of us who toss and turn a bit in the night.

This combination of factors allows many people to relegate some of their penis extender time to the wee hours — though that may not be ideal for guys who tend to make a trip to the bathroom in the middle of the night.

One downside is that Phallosan Forte lends itself to more gradual results because its tension is lower than that of Quick Extender Pro — unless you spend an extra $120 on the Phallosan Plus+ add-on that grants you additional tension and stability. Combined with the Phallosan Plus+ addition, you can get up to 4,800g of pressure, which is even more than Quick Extender Pro. However, based on our testing and analysis, we believe most people will not need 4.800g, so we suggest you try Phallosan Forte on its own before considering the additional investment. It's also worth noting that the addition of the Plus+ renders the whole penis extender unsuitable for overnight use or use under clothing, as it's much larger.

Phallosan Forte pricing

Phallosan’s pricing is very straightforward. It offers two central products:

It’s a bit disappointing that you don’t actually save any money by purchasing the Forte and the Plus+ at the same time. That said, it does make it easier to start with just the Forte and know that an upgrade won’t be more costly down the line.

Phallosan also sells an array of accessories that you’ll need to pick up from time to time. These include protector caps and sleeve condoms, which are silicone pieces that work with the vacuum pump to protect your glans and secure the vacuum seal along the shaft. These require regular maintenance (coating with baby powder every 2-3 uses), or they will degrade quickly and can become uncomfortable.

To be fair, every company has some expendable parts like these that you’ll have to replace over time. Phallosan’s parts are more expensive, but they’re also more durable if you care for them. Phallosan covers its penis extenders for two years, which is one of the longer warranties in the space (Quick Extender Pro, for example, offers a 1-year warranty).

When you first unpack your penis extender, you might understandably be a bit confused about how to put it together or get it in place. And in the case of Phallosan Forte, the device is a bit more complicated, and tutorial materials from the company are a bit lacking, in our opinion.

To clear things up, we’ve made some helpful videos that walk you through the process with the two penis extenders we recommend most; many other extenders follow similar procedures. When used as directed, penis extenders are very safe.8 You should follow the instructions on using your device to avoid potential bruising or discomfort. Listening to your body is important, too. Reputable companies explain a process of gradually ramping up tension strength and the number of hours you use the penis extender. Following these programs is a necessary step to avoid any potential bruising.

Tingling and stretching sensations are normal when using a penis extender, but pain and discomfort aren't normal or expected at all. If you encounter this, you should discontinue use immediately and contact the company. As long as you use the penis extender as intended and aren't overly aggressive in how quickly you increase tension, the devices are designed to cause no harm.

Safety in comparison to alternatives

The only other viable alternative to penis extenders at this time is surgery, which carries with it the risk of unintended side effects. Surgery is almost always a last resort in medicine because the risks of complications tend to increase as compared to non-surgical solutions. When there is a non-invasive, non-surgical method of achieving the same desired results, scientists and medical professionals almost always advise against surgery.10

Penile extension surgery is particularly scary. Some procedures involve severing the base of the penis where it is attached to the pubic bone, which allows flaccid penises to hang more significantly. But these surgeries carry various risks, including erectile dysfunction and difficulty with control during intercourse, as the lack of an anchor to the erect penis makes it hard to direct one way or another.

Other procedures, often intended to increase penile girth, involve layering extra tissue beneath the skin of the penis. This tissue sometimes comes from cadavers, and both rejection and infection are noteworthy risks. Alternative methods now exist to implant silicone instead of actual tissue; in the case of silicone implants, the surgical procedure must be reversed due to serious compilation in three percent of operations.

We tested and evaluated penis extenders based on effectiveness, price, safety, comfort, and convenience. This helped us form an overall picture of value and usability. We also gathered opinions from numerous customers who gave us their own reviews of the penis extenders as well, in order for us to develop a picture of broad user satisfaction and success.

The best penis extender for you might not be one of our top choices, but a different device, depending on a few factors in your life. Here are some major considerations.

Effectiveness

Winner: Quick Extender Pro

How quickly do you want results? Some penis extenders are built to deliver results after 6-12 months of gradual, low-impact usage. Other companies design their devices with greater tension strength, allowing for more intensive use and — theoretically — faster results (as long as the penis extender is comfortable enough to wear for long periods).

Quick Extender Pro offers models with up to 4,000g of tension strength, and even its least expensive model delivers 3,000g — 200g more than the next best offering from traction devices. The only penis extender in our guide to offer more pulling force without the addition of actual hanging weights is Phallosan. But you can only get up to that number (4,800g) by investing in the company’s most expensive package.

Your preference will depend on how quickly you feel you need results, balanced with your availability to wear the device for what could amount to an extended period each day. Finding the right balance for you will be key to your contentment with the penis extender. And, of course, you must listen to your body as you use a penis extender. If you're experiencing discomfort, it's a signal from your body that you need to slow down and reassess. Do not ignore discomfort because these devices shouldn't cause discomfort when used properly.

Since penis extenders require a significant commitment in time and a big dose of patience in order to be effective, it’s important to consider how they size up in the landscape of alternatives. That landscape happens to be pretty small, though.

It’s not hard to find sexual health supplements and topicals that will promise just about everything in their marketing, including penis enlargement. But at this time, there is no supplement or cream that clinical research indicates could be effective for penis enlargement. Certain supplements may improve the strength and durability of erections (though usually not as reliably as a prescription ED medication); by doing so, these can create an impression that a penis is getting larger, even though what they’re actually doing is something else (facilitating a healthy erection).

Surgery is the only alternative that has been shown to be effective for penis enlargement, but it comes with a whole host of risk factors, some of which actually cause the penis to shrink. At the end of the day, a penis extender is the only effective means for enlargement that has scientific support and a reasonable safety profile.

Safety

Winner: Phallosan Forte

When used correctly, all of the best penis extenders — certainly those in this guide — should be safe. Penis extension by traction and vacuum pressure have very few adverse effects. Some manufacturers sell moisturizing creams many men like to apply after their sessions, as users report some dryness being common. But beyond that, proper use is very safe.

That said, there is a gradual learning curve with these devices, along which you’ll find some significant confusion. If you make a mistake in assembly or misinterpret some of the directions, you could easily put yourself in a position of overextension. You might not even feel it at first, but you might experience some tenderness or bruising afterward.

We’ve provided video guidance for the assembly and use of the most common types of penis extenders in our “How to use a penis extender” section a bit earlier in this guide. Of all the models our team tested, the Phallosan Forte had the shortest path to mastery, and the convenient ability to adjust the tension strength by loosening or tightening the belt mid-session can help ensure safety.

Phallosan also employs a visual system to indicate whether you’re applying the right amount of tension. Pictured below, this “traffic light” system is a set of green, yellow, and red lines that reveal themselves as you increase tension. If you see yellow or red, you want to back off. Green is where you want to be.

As with efficacy, it’s important to understand that extension is a superior option to potential alternatives. Surgical intervention is significantly dangerous, and pills or creams that claim to enlarge your penis have no scientific support. Some companies upsell based on comfort and packages that include additional features to help you maintain comfort, including everything from moisturizing lotions to padding material.

Like it or not, these packages are often well worth it, which is why we often recommend you consider them above the more entry-level packages. Comfort that doesn't sacrifice strength allows you to wear the device without issue for longer periods, which in turn may speed up the process of penis enlargement.

The Phallosan Forte uses more robust comfort accessories than the typical foam and gauze you get with traction-only devices. And the adjustability of its belt allows you to dial in your exact extension after you put it on — not all devices offer that convenience or comfort. In fact, the Forte is one of only two models in our guide suitable to wear overnight, though we recommend getting familiar with the device (and your tolerance for it) before wearing it to sleep.

The one big knock on Phallosan is its lack of a money-back guarantee. In this department, Quick Extender Pro reigns supreme. It offers a 6-month satisfaction guarantee that doesn’t have the loopholes you’ll find in other companies’ policies.

Cost

Winner: Quick Extender Pro

Penis extenders aren't all priced the same. If you're on a tight budget, you may well want to ignore what we just said about comfort and opt for the device package with the lowest price tag. If that's a necessity, rest assured that you can choose an entry-level option from any company in this guide and be well-served by it. All of these products, at whatever price level, are safe and effective.

The only downside to many lower-priced packages is that you may need to use the penis extender for more weeks or months to get the same results a slightly stronger device could offer. And if you find you like using what few comfort materials a lower-tier package provides, you’ll have to invest in replacements before long, which can be more expensive than if you’d opted for a better package from the start.

Quick Extender Pro offers the best marriage of price, comfort, efficacy from tension strength, and included accessories. It does this at its most basic level, delivering 200g more tension strength than the next strongest traction-only device for about $30 less.

And broadly, penis extenders are the most affordable of a limited set of options for penis enlargement. The alternatives, which involve surgery, will be the costliest treatment paths. Surgical procedures to lengthen or increase the girth of the penis cost thousands of dollars — often ranging from around $6,000 to $20,000, depending on your location and the nature of the procedure. Often, insurance will not cover any of the cost, though you would need to check with your insurer in advance.

Let’s take a closer look at the other models that made the cut in our guide.

The combination of comfort, convenience, and affordability makes the PeniMaster PRO appealing to many men. While not as affordable as competitors like Quick Extender Pro, it is less expensive than its closest functional rival, Phallosan Forte (both utilizing a combination of vacuum and traction techniques). This could make PeniMaster PRO a major contender for your business.

In comfort, it stands toe-to-toe with the Phallosan Forte but at a lower cost. The major difference is that Phallosan offers more tension strength without needing complicated and expensive accessories. To equal Phallosan's tension strength, you'd need to purchase the PeniMaster PRO weight-pulling force generator, which essentially attaches small weights to the end of the vacuum component; those weights literally pull down on your penis. This is, by far, the most setup effort and the largest package of the penis extenders in our guide, making a fully decked-out PeniMaster PRO less suitable for travel.

One of our testers echoed the sentiments of some customers who find the PeniMaster PRO not quite as ideal as the Phallosan Forte when trying to wear it discreetly under clothing; these men felt that the only good position for it was straight upward, as opposed to downward or to the side (two options that PeniMaster contends are suitable). Most of our testers found it to work just fine in a downward or diagonal position, however.

PeniMaster pricing

If you go onto the PeniMaster site and buy only the PeniMaster PRO basic system, you won’t really be able to use it for anything other than vacuum pressure applied to your glans. The company’s other sets, which we’ll outline in a chart below, offer experiences similar to the Phallosan Forte.

PeniMaster offers more savings opportunities on its products than some competitors, including a 20% discount for students and a 12.5% discount for owners of competing products. You can’t combine any of these discounts, though.

PeniMaster’s price structure is based on its Euro pricing, so prices in U.S. dollars will fluctuate from time to time.

Here’s a quick breakdown of PeniMaster prices at the time of this writing:

PeniMaster’s 5-year warranty is the longest of any product in our guide, but the company’s 1-month money-back guarantee is lacking. You only get one month to try the product, and that’s not enough time to see results from extension. It’s one thing if you find the device too cumbersome or difficult to use, but if you get six months in and don’t see any results, that 1-month guarantee will have long since expired.

Word of caution

If you decide that PeniMaster PRO is the best penis extender for you, don't buy just the "PeniMaster PRO Basic" on its website because this includes only the bulb that attaches to your glans. By itself, this bulb is not a penis extender and will not achieve your goals of penis enlargement; it's intended to be used either with PeniMaster's rod or belt system or a different weight system of your own devising (something that isn't clear enough on the website, in our opinion).

To buy a package that will achieve penis enlargement out of the box, you need to opt for the belt or rod expander systems, both of which include a PeniMaster PRO bulb; the complete set, which includes both rod and belt options; or the PeniMaster Classic, which includes only a traditional rod-based penis extender without the PeniMaster PRO glans bulb attachment.

The same parent company, DanaMedic, produces the next three penis extenders in our guide. This company extends a 6-month money-back guarantee on its products, but some specific caveats are only spelled out in the fine print of the terms and conditions.

If you want to be eligible for any money-back guarantee from these brands, you have to do the following:

1. Sign up for the company’s Diary
2. Input your weekly measurements into your Diary profile
3. Be prepared to submit both before and after pictures of your penis to prove the device didn’t work for you.

You would likely be denied any guarantee claim if you didn't know about the Diary, which is very well hidden on these companies' sites. You might be denied a claim if you miss a week of measurements. You might be denied if you forgot to take a picture before starting treatment. And in testing, our attempts to sign up for the Diary were all unsuccessful. No verification emails were ever sent, making contributions to the Diary impossible.

All of these loopholes appear to exist in bad faith to prevent customers from having the guarantee they were promised. The closest thing to a readily available caveat on any of these company sites is that SizeGenetics lists “Our ‘money-back’ confidence guarantee,” with the quotation marks around “money-back,” as if to let you know it’s a sham.

To be fair, these penis extenders should still work for most men, but if they don't, DanaMedic doesn't appear interested in making good on its promises.

None of the other companies in our guide have such requirements for you to access their guarantees.

SizeGenetics is one of the most worthwhile products from DanaMedic, a company that makes several penis extenders, including MaleEdge and JES Extender. It’s neither the company’s cheapest option nor its most luxurious, so it offers something of a Goldilocks position in the lineup. That said, the parent company has one of the sneakiest guarantee loopholes in the business (as described in the previous section above), all but ensuring that no one will be able to make a guarantee claim if they’re dissatisfied with their results. And our testers found that SizeGenetics customer service, in particular, was very difficult to deal with and remarkably unreliable.

Just like MaleEdge and JES Extender, a SizeGenetics extender delivers up to 2,800g of tension strength, making it an effective penile traction device. One noteworthy feature SizeGenetics offers that other penis extenders could benefit from is the opportunity to make minute adjustments to your tension while wearing the device. You can achieve this by rotating both of the threaded adjustment bolts at the base of either extension arm. If you notice discomfort during a session, you won’t have to take the penis extender off and mess with the extension rods; you can make those fine adjustments instead.

The major downside we discovered with SizeGenetics is the weight of the device. Although SizeGenetics enables you to fine-tune those settings, this penis extender is one of the heavier devices we tested, likely due to the use of stainless steel over aluminum, which adds durability but also weight. The added weight, as compared to Quick Extender Pro or MaleEdge, is noticeable.

With penis extenders, lighter is definitely better, in our opinion, as long as the device doesn't sacrifice tension strength.

SizeGenetics pricing

SizeGenetics offers four packages. As you spend more money, packages gain extra extension rods and products like SizeGenetics’ moisturizer or Boost multivitamin. These products may be useful for some men, but SizeGenetics does not list the supplement facts for Boost on its website, and all that’s said about the moisturizer is that it’s silicone-based. Given these shortcomings, it might be wise for men interested in SizeGenetics to start with a smaller package that doesn’t include these extras.

Here’s a quick breakdown of products and pricing:

In one of our testing phases, we received a set from SizeGenetics that didn’t have any extension rods other than those already attached to the device. We contacted customer service and were given a significant runaround, eventually having to make a formal warranty claim that took several weeks to resolve.
